APPLICATION FOR TEMPORARY DOCK, FLOAT, MOORING OR RAFT

TO: Harbor Master, Town of Harvard

Under the provisions of the Massachusetts General Law, Chapter 91, Section 10A, I/We apply
for a 1 year permit to establish a temporary dock, float, mooring or raft.

Location of dock, float, mooring or raft
Purpose and Description of dock/float/mooring/raft: (Upload scale drawing) If moorings are requested, describe type, anchoring method and max boat length.
Please attach diagrams and any other documents to support your application
Files must be less than 5 MB.
Allowed file types: gif jpg jpeg png bmp eps tif pict psd txt rtf pdf doc docx odt ppt pptx odp xls xlsx ods.
Applicant 1 Agreement
I/We understand that if this application is approved, I/We, through the signing of this document by an authorized party or agent, shall indemnify, hold harmless, and defend the Town of Harvard and its agents and employees from all suits and actions, including attorneys’ fees and all costs of litigations and judgment of every name and description brought against the Town as a result of loss, damage or injury to person or property by reason of the granting of this permit and/or the use of this temporary dock, float, mooring or raft.
